How it Works

What to Expect from Your Medication Visit

1

Before Your Appointment

Have the bottles of your medications nearby! Call us today at 1-844-866-3735, or respond to one of our texts, emails, or phone calls.
2

During Your Appointment

A medication expert will ask you questions about your medications and health. They'll answer questions you have. Then they will give you some helpful advice.
3

After Your Appointment

We will mail you a summary of what we spoke about with you. It will have your updated medication list, their purposes, and directions. This will help you get the best results from your medications. We may also work with your doctors and pharmacy to find more ways to help.
After my MTM service, I felt better about the medications I was taking. I also got some great tips that will help me get my medications and save money.
The pharmacist I spoke with was so knowledgeable! She was really helpful and answered all my questions.
I wish this service was available years ago. My pharmacist helped cut down on the number of pills I’m taking, and some bad side effects have finally stopped. And it cost me nothing.

Frequently Asked Questions

Where did you get my contact information?

Our clinic, SinfoniaRx,  is a partner of your health insurance plan. We have permission to reach out on their behalf.

Is this program really free?

Yes! Our services are fully covered by health insurance for eligible patients. You are likely eligible if you have at least 1 chronic condition or are taking more than 5 medications. If eligible, our services cost you nothing but a little time for the phone call. Many patients who receive our services save money on their medications.

How do I know if I'm eligible?

You can call us at 1-844-866-3735. You can also call the phone number on the back of your insurance card and ask a member services representative to check for you.

How can I find out if this is a real service for me?

Call the phone number on the back of your insurance card and ask a member services representative to check for you. Our clinic, SinfoniaRx, is a partner of your health plan.

Can a family member speak to the pharmacist for me?

Yes, as long as we have your permission.

Why would I need this service if I already have a doctor and pharmacist?

During your appointment, you will speak with a clinical pharmacist. A clinical pharmacist does not replace a doctor or community pharmacist. We work with them to help you. We help patients take their medications right. We will help stop side effects and bad drug combinations. We look for ways to save you money. We also work with your doctors and community pharmacists to get you the best care. Our phone call with you is focused on what you need and your medications. Once the appointment is over, you will get an action plan for you to follow. It will also have advice to discuss with your doctor.

Am I required to participate in this program?

No, the program is optional. But it is a great service covered by Medicare Part D health plans. Our federal government requires these health plans to offer this program to members who would benefit.
